IMPORTANT INFORMATION CONCERNING YOUR PEST MANAGEMENT PROBLEM
Health and Safety Information
You must advise us of any pets or any personal or family/staff allergies, or other concerns.
Any persons who are particularly sensitive, or who suffer from allergic reactions, should, as a matter of precaution, remain out of the premises for a period of 24 hours and not return until after the premises have been fully ventilated.
You must remove or protect any people, animals, birds or fish during the treatment period.
You must cover all food items and all food utensils.
You must avoid contact with treated surfaces until the control agent has dried.
You must observe any verbal advice provided by our firm or our licensed operator at the time of treatment.
You must ensure your premises are properly ventilated after treatment. Leave any air-conditioners on 'vent mode' (so they circulate outside air into the building) for a period of at least four hours after the treatment or after re-occupying.
Water Tanks: No treatment can be carried out around water tanks not fully sealed or to water collection areas. You must advise us of water tank installations.
Payment Terms and Details of FREE Service Period
If during the course of the program it is found that structural or environmental conditions prevent us from performing any section of the Management Program then the FREE service period or the cost of your investment may have to be reviewed.
Unless specific written arrangements have been provided the full contract price shall be due and payable recoverable by this Firm on commencement of the initial treatment.
Any FREE Service Period shall be null and void unless all monies due and payable to this Firm have been paid in full within the standard terms of business above.
Where a FREE Service Period has been provided then, upon notification by you, we agree to provide you with any remedial treatment(s) as may be required to the nominated property, AT NO COST TO YOU. Any FREE Service Period is conditional upon you notifying us of signs of pest infestation.
No responsibility is accepted, or warranty implied, for any damage or consequential losses that may occur as the result of past, current or future pest activity.
The proposed treatment only applies to the pest species named overleaf on this Pest Management Proposal. The program does not provide protection against any other pest.
Treatment details and control agents
Only control agents approved by the Department of Primary Industry and Energy's Australian Pesticides and Veterinary Medicines.
Authority (APVMA) and registered for use by the relevant State or Territory Registration Authority will be used.
While every care is taken, this firm will not be held liable should staining of timbers, fabrics, wall coverings or any other articles occur.
WARNING. Where drilling and or cutting is required; no liability is accepted should damage result to concealed services such as power, gas, water, etc. You indemnify Us of any costs that may arise from such possible damages. Clear and accurate plans should be provided by you before we commence.
You accept that this pest management program can be rendered ineffective if you fail to implement our recommended hygiene procedures, by making building alterations, or by the introduction of conducive or infested materials.

Termites and Other Timber Pests
This program does not cover treatment for, or inspection for, termites (white ants) or any other timber pests.
We strongly recommend that you should have your property inspected for termites at least once every twelve months.

1.Proofing:
1.1 Bristle strips to base of door to prevent rodents and insects entering.
1.2 Bird netting to areas to prevent birds nesting and roosting.
1.3 Bird spikes to prevent birds roosting on ledges.
1.4 Base of door damaged resulting in gaps that may provide potential pest entry - repairs required by client
1.5 The door frame gaps need to be sealed to prevent pest access - repairs required by client.
1.6 Damaged fly screen needs repairing/replacing.
1.7 Fitting a fly screen to the window/door to prevent possible access for flying insects.
1.8 Gaps around pipes need to be sealed to prevent pest entry
1.9 The strip curtain door damaged needs to be replaced to prevent flying pest access.
1.10 Repairs to tiling required sealing off pest harbourage and breeding areas.
1.11 Window broken and needs repairing to prevent flying pest access.
2.Benefits:
The Benefit of proofing to areas identified in reports is Control and reduction of pest preferred harbourage, breeding and ingress points. Controlling and reducing pest activity within these preferred areas will assist in providing a controlled environment to your premises.
2 Housekeeping:
2.1 Staff leaving windows open will allow insect ingress.
2.2 Bird droppings/old nesting material needs to be removed.
2.3 Drains need cleaning to avoid problems with insects.
2.4 Guttering is blocked and needs cleaning.
2.5 Deep cleaning of production machinery is needed.
2.6 Dust and debris/spillage need to be cleaned-up.
2.7 Cleaning of the production/preparation area needed.
2.8 Thorough cleaning of the preparation area is needed.
2.9 Spillage of liquids/products needs to be cleaned regularly.
2.10 Overgrown vegetation around building/fence line needs cutting.
2.11 No access possible, arrange access to be provided on the next visit.
2.12 Additional internal baiting points are needed in this area.
2.13 Additional External baiting points are needed around the building perimeter/ fence line.
2.14 Additional Rodent bait stations are required.
